• I understand that my participation in the program is completely voluntary, and that I can withdraw from this program at any time without any negative consequences. I understand that if I decide to withdraw from this program my client/s will still have access to the OCD? Not Me! treatment program.

• I understand that OCD? Not Me! will be provided as a fully online automated computer program.
• I understand that OCD? Not Me! is an online program and therefore does not provide additional advice, guidance or follow up by telephone, email or other means of communication.
• I understand that any information collected by the program will remain confidential and client information will remain anonymous.  I also acknowledge that information collected may be used to report on the outcomes of the program but will be in no way identifiable.
• I have read and agree to the Terms and Conditions of participating in this online program
